{"id":1777,"date":"2014-12-22T20:14:05","date_gmt":"2014-12-22T20:14:05","guid":{"rendered":"http:\/\/www.abetteremergency.com\/blog\/?p=1777"},"modified":"2017-01-19T17:59:09","modified_gmt":"2017-01-19T17:59:09","slug":"saving-lives-at-active-shooter-events-is-in-the-publics-hands","status":"publish","type":"post","link":"https:\/\/abetteremergency.com\/blog\/2014\/12\/saving-lives-at-active-shooter-events-is-in-the-publics-hands\/","title":{"rendered":"Saving Lives at Active Shooter Events is in the Public&#8217;s Hands"},"content":{"rendered":"<p>In a perfect world police are there to handle criminal acts, fire fighters available to perform rescues, and EMS personnel close at hand to apply life-saving interventions. \u00a0Unfortunately, that is not always the world we live in.<\/p>\n<p>We need to have the conversation on how best to address the &#8220;planned&#8221; response to the aftermath of a violent criminal act. \u00a0The data tells us it could occur anywhere &#8212; at work, out in public, a recreational facility, sporting event, place of worship or healthcare facility. Unless it occurs at a police, fire, or EMS station, the first people on scene will be members of the community. Why don&#8217;t we train them so their reaction is not random but based on tasks designed to positively influence survival?<\/p>\n<p>There are a number of mitigation strategies organizations and facilities should undertake in cooperation with their <a href=\"http:\/\/www.washingtonpost.com\/local\/crime\/trauma-kits-help-police-control-severe-bleeding-saving-lives\/2015\/01\/26\/a6d0dcde-8ade-11e4-8ff4-fb93129c9c8b_story.html\">local police<\/a> and emergency management agencies. \u00a0This can include training their staff in how to respond to this type of situation &#8212; a program similar to people learning CPR. \u00a0Lets call it community-based hemorrhage control for lack of a better term. Science tells us that the people whose lives are saved at the next attack will be by someone who can apply a tourniquet within minutes of injury.<\/p>\n<p>Consider this case study from the Boston Marathon bombing. \u00a0\u00a0A 34-year-old man was brought to an emergency department at a hospital suffering from multiple traumatic injuries which included a complete amputation of his leg below his right knee. \u00a0A tourniquet had been applied to the right upper leg by prehospital providers but was not adequately tightened to control the bleeding. \u00a0At the hospital the tourniquet was tightened, and a second, military-style tourniquet was added which stopped the bleeding.<\/p>\n<p>Tourniquets work and the risk of complications from aggressive and unnecessary use is outweighed by the risk of not controlling bleeding in situations like these. \u00a0The public is trained in CPR. We see Automated External Defibrillators (AEDs) in most large buildings and venues, all designed to help save lives from heart attacks. \u00a0Granted, more people die of sudden cardiac arrest than bleeding to death from a gunshot wound or blast injury from an Improvised explosive Devices (IEDs, but there is a need for this training.<\/p>\n<p>I&#8217;m advocating all citizens learn how to <a href=\"http:\/\/www.abetteremergency.com\/blog\/courses-presentations\/active-shooter-life-safety-strategies-and-tactics-for-civilians\">improve survival in active shooter situations<\/a>\u00a0and to use hemorrhage control kits that are placed in venues alongside AEDs.<\/a> \u00a0<a href=\"http:\/\/abetteremergency.com\/blog\/wp-content\/uploads\/2014\/12\/2175386.jpg\"><img decoding=\"async\" loading=\"lazy\" class=\"alignright size-full wp-image-4060\" src=\"http:\/\/abetteremergency.com\/blog\/wp-content\/uploads\/2014\/12\/2175386.jpg\" alt=\"\" width=\"250\" height=\"250\" srcset=\"https:\/\/abetteremergency.com\/blog\/wp-content\/uploads\/2014\/12\/2175386-150x150.jpg 150w, https:\/\/abetteremergency.com\/blog\/wp-content\/uploads\/2014\/12\/2175386.jpg 250w\" sizes=\"auto, (max-width: 250px) 100vw, 250px\" \/><\/a><\/p>\n<p>Like it or not, members of the public will be first on scene of the next sick attack by a gunman or the detonation of an IED. Why not offer training designed to change the outcomes of the wounded?
